Use the Medication record to document details about medication that a client is taking.

You can use the Add Item button to document more than one medication, but ALL the medications listed will need to share the same start date and end date.   To show a different start date and end date for each medication, enter a separate Medication record for each one.

To show that the client is no longer taking the medication, uncheck the Currently Taking Medication checkbox.  This will bring the End Date and Reason Discontinued fields into view so you can record that information.

Tip:  select the Medication Order checkbox to expand that section if you wish to record that information.
